When I drive my car (97 ford explorer/136,000 miles), it hesitates when I accelerate at low speeds.?

When I drive on the highway its fine, but when I stop frequently at lights, and I accelerate, it feels like someone just put it in neutral. The rpm's go up but it just hesitates. It just hesitates for a second and then it jerks and goes. Any suggestions on what it may be?

Update:

I don't have a lead foot, I accelerate normally.

    sounds like it maybe low of transmision fluid.as RPM's go up it pumps fluid up then it will jerk.

    dont put the rpms so high. u want to gradually accelerate.
