Insurance Company v. Insurance Agency?

I got in an accident yesterday and now I have to fill out a paper with my insurance information. There's a space asking for the name of my insurance company and says "not agency" in parentheses beside it. My insurance is with State Farm, is that what they want me to write?

    State Farm Mutual Automobile Insurance Company is the insurance company unless you live in NJ, then it is State Farm Indemnity Company

    In Texas it is State Farm County Mutual Insurance Company of Texas

    you should write State Farm Insurance (they don't want you to write Brown Insurance Agency) an agency writes policies through

    several companies, the authorities want to know WHAT company
