exercise for beginners...?

I lost 70 pounds in the past (about 5 years ago) with diet and exercise! However, I have gained that weight back and an additional 35 pounds. I just decided to get back on track! I have the diet part down (6 small meals per day) but Its like I can't remember how to exercise... Its pretty sad. What should I do to ease my way back into working out. The only cardio I can handle at this point is walking. I also have a couple 25 pound dumbells. Any advice will be appreciated. Thanks! Fyi: I am a male, 6'4 and 400 pounds
