Yes in the Marine Corps we trained in hand to hand, with our bayonet in our hands, and with the bayonet attached to our rifles.

Basic Army melee weapons include the bayonet and rifle butt/stock/barrel. There is pugil-stick training, which is a basic type of melee/hand to hand training technique. But as far as extensive training in the general use of melee weapons, no, as weapons like say a mace or swords would be unauthorized as actual weapons for combat use (even though swords are part of the dress uniform of certain types of soldiers, like the cavalry or marines)

If you wanna run around with a sword and shield, you should stick to warcraft.

Yep. It's called "Grab the nearest weapon and beat the crap out of the enemy" or more commonly know as "Hand to Hand Combat".

Yes. There are drills involving melee weapons.

What else would one use if they are out of bullets?

In the next war (with China) there will be no prisoners taken on either side. It will be like the movie "Enemy at the Gates," except it will be waves of Chinese that will just keep coming.

To occupy and hold territory an army must send in ground troops. Ground troops unlike bombers, warships and fighter jets meet the enemy "face to face" and do not have the luxury of killing from long range. Nor are they has heavily protected as a tank. So even a child with a well-placed shot can kill a soldier. To clear a house or village soldiers and Marines must go from corner to corner, house to house to flush out an enemy. This principle is as old as warfare. Even the the best trained armies can be bested by a poorly trained one in an urban environment (like Stalingrad). Still the US casualties are low compared to other wars. In the late 1940's after Hiroshima and Nagasaki US strategists declared an end to conventional war. Then along came Korea where Nukes were of no use whatsoever and the Marines and Army grunts were sent in to dig out the enemy with rifle and bayonet.
