are hospitals, labor unions, and public utilities exemption run state governmental or federal?

The understanding of whether the exemptions to the antitrust laws have me a little baffled as to whether they are state governmental controlled or federally controlled. Can someone help clear the misconception?

    Hospitals, labor unions, and public utilities are franchised to operate within the guidelines of respective state governments while maintaining federal mandates of the US Code and US Constitution.

    Increasing complexity in the marketplace has yielded a climate of circumstances where larger entities use state franchise to skirt law. When not challenged by community of leadership some entities become corrupt in their own special interests.
