What does this mean: Was hast du in den Sommerferien vor?

What is it asking, not what does it literally mean

Also, should I answer in the future, present or perfect tense?

    What are you doing for the Summer Holidays?

    Answer in the future tense. Or copy the question " Ich habe vor, nach Deutschland zu reisen" I intend to travel to Germany.

    I think in this case, in German, like in English, it would sound more natural in the present tense "I'm going to the US", but if this is for a class, your teacher may be expecting future.
