Sounds like you're studying pH phenomena. Let's take a look at the dissociation of water into H^+ and OH^-

EQUILIBRIUM, at which point the forward reaction is occurring as often as the backward reaction. At this point of the reaction, at equilibrium, the question asks you to determine the concentration of hydrogen ion.
In strong acids, which dissociate completely into their positive and negative ions, equilibrium is reached very quickly. On a chemistry problem, you'll probably be given the concentration of an acid. The equilibrium concentration of hydrogen ion will be the same as the concentration of your strong acid.
Now you can take that concentration of hydrogen ion, plug it into the equation of -log[H^+ concentration], and calculate the pH.

everything that you said is technically correct. a hydrogen atom is composed of a proton (a positive particle in the nucleus) and an electron (a negative particle) that orbits the nucleus. if the hydrogen atom loses an electron, then it becomes a positively charged ion. The electron that is lost is usually attached to another atom, which then is now a negative ion because it carries an extra electron (remember the electron has a negative 1 charge).
