adulterated, when used to refer to food products, means something has been added (or not excluded) that is inedible or could cause illness or injury.

it may mean that a restaurant didn't cover their food, and insect parts/bodies were evident, or that the ceiling tiles flaked down into the prep area, or that cleaning chemicals were being stored near dry ingredients (flour, sugar) and the vapors contaminated the food.

avoid that restaurant if you can! most health depts will do a follow-up inspection if conditions are bad enough, to ensure that preventive measures have since been taken.
