me if you are in 718....
otherwise you can call your local phone provider... if it is Verizon they will charge you close to $120 (at least thats there rate in NYC) if it is a CLEC or reseller, they usuallay charge $65-75...
you can also look on craigs list for independent telecom technicians...
if you have a basic understanding of wiring (if you can wire a speaker wire) then you can do it yourself...
you would need a jack ($5 at home depot, or $1 at an electrical supply place) and some wire (cat 3 is voice grade, cat 5 is data grade, but can be used for voice)
then you just need to connect the green/red wires on the jack to the blue/white wires on the phone wiring... and use a T-18 or T-25 stapler to staple it to the wall... if you need to drill a hole to run the wire from one room to another you should also get a 1/4 inch x 18 inch drill bit....Source(s): email@example.com

Verizon wire mantinence doesn't cover new jack installations.
However installation is simple. Basically run a new set of wire from where the service comes into your house and wire it to the color coded terminals on the back of the jack. (You can wire jack to jack in a loop through the house, but this is not the best as that if one jack/wire ever goes bad, everything wired after that jack/wire will also cease to work until the bad jack/wire is repaired.)
This installation may require fishing the wire through a wall or through parts of your house and then eventually cutting a hole in the wall to install the jack. If you don't like this idea there are plenty of contractors or electricans that can add one for you.

You can sign up for inside wire maintenance for a flat fee each month. Usually this is about $5.00 per month. This covers all of the phone wire past the point where the phone co physically connects their line to you house. Depending on what your carrier includes with this service, they may install a new jack for you for free with this service. There is no commitment for this service so you can cancel it after they install your jack.

you could do it yourself... its pretty easy... just a lot of wires... but a phone jack from like Home Depot and read the instructions .... plus look it up how to online... of course thats to replace one... not to put in a entirely NEW one... for that the phone company is the ONLY way to go because they have to have access to the Phone line box.
