what information does an investment statement contain?

for example, would an investment statement for a high networth client include the portfolio structure, realised and unrealised profits, etc?

Well, it would primarily contain current positions - and yes, grouped by type. To be helpful, it should also contain information on gains/losses, although closed-out positions may not show for time periods after that in which they occurred - except perhaps on end-of-year and of course tax statements. And for certain it should show all transactions that occurred during the time period which it is reporting.
