how do I find an outline map of folded, dome, volcanic, and fault block mountains in the united states.?

These maps with links will help you get started. They outline and discuss the physiographic provinces of the U.S. (not counting Alaska and Hawaii--which you should count).

You may have to read a bit to find out which mountains formed in which ways. Just to get you started--Basin and Range is fault-block; Cascades are volcanic, Appalachians are folded, etc.
