Personal Affidavit normally written at the top of the document with the persons name, ex. Affidavit of John Doe. If you are taking testimony and having a court reporter record it to make into transcripts it is called deposition.

If it is a sworn statement (e.g. something you would have notarized), then it would be an affidavit.
