Anyone know how to make a blank version of this PDF file document?
Anyone know how to make a blank version of this PDF file document? I'm having a difficult time doing it on my own. I want to be able to have a blank version of the checklist and then add to it text and additional stuff to the file every month or so.

You would need to create a form file using Adobe Acrobat. Why don't you just download OpenOffice.org? You can create a spreadsheet document, modify it, and create a PDF from within the same program.

You may be better off re-creating a blank version in Microsoft Word (or if you don't have that, you can download OpenOffice for free from www.openoffice.org) and edit that as you need it. It looks like a reasonably straightforward table-based sheet. The trouble is that unless you have a PDF editor (which you may have to pay $$$ for), you can't directly change PDFs.
