Conclusions

Based on the information available from the self-report of these 23 women and the observation of their function by RN data collectors, attaining a level of function that will support independence is possible for the majority of older women who are residing independently prior to a hip fracture. The majority (78%) of these participants were able to perform all Katz ADLs independently after fracture, at a time period ranging from 6 months to 4 years postfracture. When the IADL scores were added as a more sensitive measure of the ability to live independently, six (26%) achieved a maximum sore of 30 and 19 (84%) scored 20 or higher. The combined score of the ADL and IADL measures was significantly related to the actual living situation (independent or assisted living) of the participants, demonstrating that these measures performed as intended. As in any situation, there are numerous variables related to outcome. Some of these not known for this sample, for example, include personal attitude, social support, and level of health care available. Age had a moderate relationship with ability to function independently, demonstrating its importance in the mix of variables.

How the study participants\' falls occurred provides some basis for teaching about fall prevention. Most fall prevention guidelines relate to environmental factors within the house, especially the bathroom. We found that 60% of the participants fell in the home, with the greatest number of those falls being in the kitchen. Examples of their fall descriptions include ‘tripped on a shoelace’, ‘was getting up from a chair’, ‘tripped over an afghan (rug) that was over lap when getting up from chair’, ‘feet were asleep when getting out of bed and fell when tried to stand’, ‘tripped over a flower pot in the yard’, ‘fell while carrying laundry’, ‘fell while making bed’, and ‘fell in a dark room’. Using such examples as these along with the points generally included in fall prevention programmes would seem appropriate in teaching older adults.
