    In this paper, we attempt to contribute to an understanding of traditional beliefs and practices regarding pregnancy and childbirth among Thai women in Northern Thailand. In particular, we examine the following issues: women's embodied knowledge of pregnancy and birth, women's explanations of precautions during pregnancy and birth, and preparations for easy birth and the role of a traditional midwife in Thai birthing care. We also look at whether women take traditional beliefs and practices seriously. We conclude the paper with some interpretations of women's accounts and implications for birthing care.

    Non-drug treatments

    Most uncomplicated UTIs in young women are a result of infection with E. coli, an organism that inhabits the large bowel. Proper feminine hygiene is essential and there may be a need to discuss matters such as wiping technique (always wipe from front to back) and avoidance of feminine sprays and scented bath products, as these may exacerbate episodes of cystitis by causing urethral irritation. Showers are thought to be more hygienic than baths.

    Other risk factors include new sexual partners and the use of spermicides, and appropriate advice (such as washing the genital area before and after sexual intercourse) should be given to the patient (Fihn et al, 1985). Advice always includes increasing fluid intake, although it should be noted that if antibiotics are prescribed, very dilute urine can affect their efficacy (Dawson and Whitfield, 1996).

    Cranberry juice

    A Cochrane review of studies into the effectiveness of cranberry juice and/or cranberry tablets identified two random control studies.

    The studies showed that the use of cranberry products significantly reduced the incidence of urinary tract infections in women at 12 months. The studies do not state the amount of juice advised, or whether cranberry tablets were more effective.

    It has become common practice to advise patients to drink cranberry juice (up to 300 ml/day), the theory being that both cranberry and blueberry juice contain a high molecular weight component that inhibits the adhesion of E. coli to the cells lining the urethral tract and bladder mucosa. This research is still being debated, and properly designed trials with relevant outcomes are neededto confirm best practice (Jepson et al, 2004).

    It must also be made clear to patients that this is a possible preventative measure and not a treatment.

    Men should be referred if they have two recurrent infections.

    Management of UTIs

    Uncomplicated UTI

    An acute uncomplicated UTI is an easily curable bacterial infection. The guidelines published in 1999 by the Infectious Diseases Society of America recommended the use of trimethoprim as the firstline treatment for acute cystitis, as did the UK Guidelines issued by the Department of Healfh (1997). The use of this antibiotic seems to have declined in recent years, possibly because the use of broader spectrum antibiotics as a first-line treatment is now more common.The society and the

    Department of Health Standing Medical Advisory Committee (SMAC) recommend that this practice should be avoided as multi-resistance is increasing.
