See the list of required documents for a visa holder to enter the Schengen Area, they are identically for most countries. Those who do not fulfil the requirements may be refused entry even if they hold a visa.

Please note that a visa is not a guarantee for access. The final authorisation remains with the Immigration Officer at the border.
• In addition to a valid passport with a valid visa (except for exempted nationals), when entering, a traveller should be able to produce the following documents to the Immigration Officer:
𐁸 For tourism: hotel reservation, other accommondation details or documents from a travel agency
𐁸 For a professional/business visit: letter from the employer, invitation from a business partner or organisation
𐁸 For a private visit: invitation letter or contact details from a family member or friend living in the country
• Concerning his/her Means of Living
𐁸 Cash, traveller's cheques, valid international credit card
• Proof of Travel Insurance
𐁸 Covering all type of expenses including doctors and hospital fees as well as repatriation costs
• Guarantees of Return
𐁸 Air ticket, ferry ticket, road or other travel arrangements.

But authorities are whimsical and can refuse entry to anyone they want. You could have the same name or look like someone the authorities are after, or they could just trump up something just because they feel like it. It's not done that often because they usually want your tourist dollars, but it can and does happen.

They don't need any. If you look like you are entering on a tourist visa but they think you are trying to enter to stay and work then you aint comming in.

Hotel reservations or some other proof of holiday residence would help.

there are several legal grounds for refusal of entry - it is not done on a whim.

if the visitor has NO proof that they will go home at the end of their visit -
if the visitor does not have sufficient money to live on while they are visiting
if the visitor has no ties to their home country and therefore no reason to go home
if the visitor is very pregnant (women obviously) then she will be denied entry because she obvisouly wants to have her baby in USA.

any and all of these are valid reasons to refuse entry to any visitor

If you look suspicious, if you have tatoos that might imply gang activity, you could get denied. Remember, just because a country's Embassy gives you a Visa doesn't mean that their authorities at the actual port-of-entry HAVE TO let you in. They could deny you entry and have their reasons for doing so, no matter how stupid we think those reasons are.
