    Both.

Exhaust System's have Mufflers, which make the noise quieter.

Performance mufflers make them less quiet.
No muffler, which is illegal, make it very loud.

And then engine parts change engine sound to make it louder like Intake, Intake manifold, exhaust manifold.

the exhaust because it depends on the metal, thickness of the metal, and size of the exhaust. the engine can be loud, but most aren't.

cherry bomb muffler, flowmaster exhaust, V8+ engine. it all contributes, but it's mainly the muffler.

The aftermarket Cold Air or Short Ram intakes do away with the boxes and baffles that quiet down the intake noise and the exhausts typically have less restrictive and more noisy mufflers.
