Large parts of rural India are grossly underdeveloped (no electricity, water - sewage - health care - infrastructure - educational - poverty issues), all in line with what is usually termed "third world". To me the fact that metros are semi-developed does not change the overall picture. It is typical for India that there is few solidarity with the poor (the country is highly segregated into groups by religion and social status) and and anyone middle class you talk to will tend to see only his own situation and reject to identify India with the third world term.
India calls itself a "developing country" which would be the more politically correct term to use.
