the guest in this story is Death.
The young man in the story had chosen the path of death after experiencing the refusal of a women who he deeply devoted to. The dinner is reserved for their date if the women accept his invitation, but instead, it is his last supper. When the coffee had come, he drop a tablet of some sort, presumably poison. When the doctor announced "announced the visible presence of the young man's guest", he's acually announcing his death.
